A city epidemic of 41 cases of tuberculosis was first reported in 1983 emanating from a single index case who worked as a part-time barman in a city centre public house. It has now been possible to identify a further 10 cases. The connections to the index case have been traced and found to fall into four groups: family and friends, bar staff, customers to the public house and contacts who never visited the public house themselves. Specific places of work and study away from the public house are considered as foci for the spread of infection. Only 13 of the 51 patients had had BCG prior to developing the disease. The importance of continuing BCG vaccination is encouraged, as is further enquiry into the duration of protection afforded by BCG.
